As Dakota struggles to survive in jail and rejects increasingly attractive plea offers, his public defender, Deb Sanders, believes there is more to the story. While she fights for resources to challenge the medical evidence, Guest continues his celebrated career, teaching medical students and receiving awards for his work protecting children.
Everything changes when Guest’s infant granddaughter, Beth, suddenly suffers the same constellation of symptoms that Guest has spent decades identifying as abuse: subdural bleeding, brain swelling, and retinal hemorrhages. As Beth fights for her life, Guest finds himself confronting a terrifying possibility—his own daughter, Evie, could become the target of the same system he has helped build.
Act II Beth dies, and suspicion quickly falls on Evie. Watching investigators and child abuse specialists apply the same assumptions he once taught, Guest begins questioning the certainty behind his field’s conclusions. He seeks out Dr. Ana Patel, a pediatric expert who believes Beth’s injuries were caused by severe infection and sepsis rather than abuse. Patel explains that serious medical conditions can produce the same findings long considered diagnostic of shaken baby syndrome.
Desperate for answers, Guest reaches out to Deb Sanders, an attorney he has spent years opposing in court. Deb challenges him to confront what his certainty may have cost others. Guest reviews Mabel’s records with fresh eyes and comes to a startling conclusion: her death was likely caused by infection, not violence.
The realization forces Guest to reexamine the foundations of his career. He begins reviewing old cases and discovers troubling inconsistencies. As his doubts grow, his hospital suspends him, colleagues distance themselves, and professional organizations begin investigating his conduct. Former allies urge him to stop digging, but Guest can no longer ignore what he has learned.
Meanwhile, Dakota endures violence and isolation in jail but refuses to plead guilty to a crime he did not commit. Armed with new medical opinions from Patel and Guest, Deb prepares for trial.
Act III As the case approaches trial, the prosecution’s confidence falters. Plea offers shrink from twenty years to ten, but Dakota refuses them all. For the first time in his life, Guest agrees to testify for the defense.
On the witness stand, Guest admits he was wrong. He explains that after reviewing additional records and current research, he can no longer state with medical certainty that Mabel was abused. Under a brutal cross-examination, prosecutors attack his credibility, his career collapse, and the investigations surrounding him. Guest remains firm, acknowledging that certainty and truth are not always the same thing and admitting that Dakota has paid the price for his mistakes.
The jury returns a verdict of not guilty, and Dakota walks free. In a brief but powerful conversation afterward, Dakota tells Guest that he never needed him to save him—he needed him to stop being so certain.
In the aftermath, Evie is not charged, though the threat remains. Guest’s reputation and career are permanently damaged, but he commits himself to examining the cases he once helped prosecute. The film ends with Guest visiting Michael Lerner, a man who has spent decades in prison after Guest testified against him as a young doctor. Looking across the table, Guest finally says the words he has spent a lifetime avoiding:
“I think I made a mistake.”
The story closes on a man beginning a difficult reckoning with the consequences of certainty, and a justice system forced to confront the limits of its own confidence.
